Login Instructions

To login: Enter the user ID which is the email address you requested the application be forwarded to. 

If you are logging in for the first time or have forgotten your password and need it reset, please enter your email address and click "First Time Login".  You will receive an automatic email message containing a link that will allow you to set your password.  Your password should be 8 alphanumeric characters, include upper and lower case, with NO special characters.
